Certificate of Stillbirth

Did you know that the only document you will receive if your child was stillborn is a Fetal Death Certificate? You are not issued a birth certificate unless your child is born live. This is very difficult for some parents as they are left feeling that their child never existed. You are left wondering how it is possible for someone to have died if he or she never lived. You know that your child was born, yet there is no acknowledgement or documentation of this event.

Fortunately many states, including Ohio, have recognized this issue and started offering these parents a Certificate of Stillbirth. The Ohio Department of Health, Office of Vital Statistics website has been provided below for information on how to request this document. We have also included a copy of the application form for your convenience.
